Wondering if she's had this before...at least one year ago

Did this patient survive?  so far...
Was there an official medical diagnosis of Blastomycosis?  yes, urine test showed moderate levels of antigens on Aug. 30, 2019
How and where do you think this patient caught Blastomycosis?  could've been our backyard, torn up, or a kennel with woods/streams last November.
Symptoms  cough, eye involvement, back legs lameness, possibly infected tooth which was pulled.
Treatment fluconazole and doxycycline   
Cost of Treatment (optional)   so far, around $500 for diagnosis and drugs. If tooth extraction counts, add another $638.37
