Blastomycosis Survey & Google Earth Locations

I have recently started a website which includes Survey questions about Blastomycosis incidents.
The database includes lat and lon locations so that the geographical locations of the incidents can be viewed in Google Earth.

The survey questions are still under development and your help would be greatly appreciated if you could give me suggestions for more questions that would be useful for visitors of the database.

Feel free to add records to the database, it is configured so that when updates/additional questions are added the original data input will be maintained.

The intent of the database is to provide a means for people to see where incidents are occuring, to review attributes of the area of incidents and perhaps even correlated attributes of occurances.

Please add locations and information you know of into this database so that the information will be available for anyone in need

latitude & longitude are to give an indication_to_area's where incidents are occuring.

incident locations would serve useful for awareness, for example:
recentaly the electrical company ran a new underground power line around the road loop shown in

Google Maps.

the points shown in the mapping applications were extracted from the location database

awareness example:
the underground cable contractor business is located approx 60 miles from the location of incidents displayed in the above url's

weeks after contract completion, a worker becomes ill and he is not aware of incidents in an area that he worked,
Blastomycosis might not be considered during doctor visit

whereas if a doctor or worker knew of Blasto incidents in area's that were worked,
Blastomycosis would be considered as a cause of the illness.

anything
